清光绪二十八年（1902年），北京同仁堂少东家、乐家第12代传人乐镜宇（电视剧《大宅门》主人公白景琦原型）捐山东候补道来到济南，逢故交山东巡抚杨士骧，受其委托筹办“山东官药局”。光绪三十二年（1906年）位于院前大街的“山东官药局”成立，乐镜宇出任官药局掌柜。光绪三十三年（1907年），乐镜宇注资2000大洋，购得了山东官药局承受权，更名为“宏济堂药店”。

宣统元年（1909年）,乐镜宇感悟趵突泉东流水云雾润蒸，甘甜清冽，聚精纳气之灵秀，在济南趵突泉畔东流水设“宏济胶厂”。鉴于当时生产的阿胶皆有腥秽气味，乐镜宇于是大量查阅古医籍药典，遍访制胶名家，并不惜重金从阳谷县聘请制胶奇人刘怀安来济南。乐镜宇对阿胶质量要求很高，不怕费工费料，必求精良。宏济堂东流水阿胶采“天下第一泉”泉水之菁；山东独有德州黑驴之新鲜驴皮；配以多味名贵药材；独创“九昼夜精提精炼法”（非物质文化遗产）；了除了常法熬胶之腥臭气味，胶块色如琥珀、清透甜韵、鼓舞气血，更易吸收，疗效更宏，成就宏济堂东流水上品阿胶。

1909年，进贡隆裕皇后，被清皇宫奉为珍品“九天贡胶”；

1914年，宏济堂东流水阿胶获得“山东全省最优等金牌褒奖”；

同年，又获得铁道实业部颁发的“超等”奖状，此乃当时华夏制造业之最高褒奖；

1915年，宏济堂东流水阿胶获巴拿马万国博览会优等金牌奖，被时人誉为“国胶”，成为中国第一个走向世界、在国际上获得金奖的中药制品。

据《济南中医药志》载，宏济堂东流水阿胶“有福、禄、寿、财、禧等12种型号”，产品成色之佳、疗效之宏都超过了东阿、阳谷产品，宏济堂东流水阿胶在国内销于上海、广州、浙江、福建、安徽等省，在国外行销马来西亚、新加坡、印度尼西亚、日本等国。当时阿胶市场几为宏济堂东流水所独占。受到清廷官员、贵胄、达人及清末民初袁世凯等当权者、中华民国先驱孙中山、中华人民共和国缔造者毛泽东等代代崇用。

宏济堂东流水阿胶的成功促进了宏济堂事业的发展，从1910年起，乐镜宇先后在北京前门外大街蒋家胡同建立了“济南宏济堂参茸阿胶庄”；在济南商埠建立了宏济堂西号，在济南经二纬一路建宏济堂中号，在济南西关开药栈等。上世纪三十年代，宏济堂总收入已占同仁堂总号的三分之二，济南宏济堂也与北京同仁堂，杭州胡庆余堂并誉为“中国三大名堂”。乐镜宇因而也在掌管济南宏济堂的同时，主掌北京同仁堂。

宏济堂东流水阿胶厂的部分房间在1925年—1927年，还作为山东省委机关所在地，主要领导人都在此居住或办公。

建国后，年事已高、多病的乐镜宇仍然受到毛主席、周总理的邀请，由其侄子乐松生代为荣幸受到毛主席、周总理的二次接见。

1955年7月，济南市人民政府对以宏济堂为龙头的全市所有中药老字号进行公私合营改造，成立了济南市药材公司（济南药材站），1996年改制为济南药业集团有限责任公司，即现在山东宏济堂医药集团有限公司的前身。

2008年，为了挖掘珍贵的中医药产品，恢复传统的宏济堂东流水阿胶生产技艺，山东宏济堂医药集团有限公司投资成立“山东宏济堂阿胶有限公司”，有针对性地调配人员、寻访和重用仍然健在的老药工、老技工，传授宏济堂东流水阿胶制作技艺，对这一宝贵的中药产品进行保护性开发。经过近年来的努力，山东宏济堂阿胶有限公司按照老传统、老工艺、老配方、老选料，“九提九炙”、纯手工制作的宏济堂东流水阿胶在中断近60年后恢复生产上市。2010年10月，央视CCTV4《走遍中国》拍摄的《复活的古胶》在世界170个国家播出。去年以来，央视CCTV4、CCTV7、CCTV12频道又先后播出了《走遍中国——黄马褂》、《国宝档案——九天贡胶》、《乡土——小楼春秋》、《见证——古胶传奇》等反映宏济堂东流水阿胶历史文化的专题纪录片，对这一极具历史文化特色产品进行了充分的肯定和深度报道。
